It can even very frustrating when filling out forms . For example . I have to go to a new clinic , because I moved and my old clinic is too far. When going to new clinic we have to fill out a information form , so they have an idea of whether or not we are on meds, health related issues, our name and ect. Your old clinic should send copies of your medical history to your new1. I have to do similar w/ whom I have been seeing for years. I don't see him often though and what I might put might be diff. from what Iput when last saw him, except for anything to do w/ my meds.

Hi RandySherman, You are definitely correct on reduction of reading and writing skills. Seizures have strong influence on them as well as age and medications for this retiree. Inability to use arms and legs is also increasing in such things with more inactivity courtesy of PCs, tablets, and smart phones. These happenings in life can also bring on difficulties in doing games, you found easy growing up.
Filling out a form in a medical office can be a little different in each office. Although @A MyEpilepsyTeam Member is correct, they should be able to send the information from one to another, with different computer systems and languages, plus alternate medical beliefs and practices, it appears to be difficult. Almost like these cashiers not able to process what your buying with the scanner and/or system done in stores.

I sometimes struggle with motor skills, but that doesn't bother me nearly much as sometimes having to really think and focus on somethings. Not motor skills, but like I have to think, ok what do I have to do again? It's scary getting slower at times, and forgetful, but I'm still blessed with many other things in life and health!

When registering in a new clinic or medical practise, the forms seem to take forever to complete... I am lucky and tend to remember most of my epilepsy history, i.e. medications, dosages etc.

I have since downloaded an app on my cell phone, and entered all my Emergency Data, i.e. medications, etc. just in case the information is needed urgently.

i struggle all the time. when i go to a new dr, i take my parents with me. my parents write down what i say on that clipboard.
