Is this an issue (for residents of the United States) because you are not living close to one of the large cities with major hospitals or medical facilities? Or do you know of other reasons?
And for you who do not live in the United States, do you have a similar problem in your country?

My primary doctor referred my neurologist but because of the virus we have only had webcam appointments. I live in a city so my next appointment may be in his office depending on this virus,
I got lucky. When I was in Tennessee I had a really good doctor there. But it was because my Gastroenterologist fought for me to have a good Neurologist. In Ohio I'm fighting for myself. My Primary care doctor wanted to be the one to handle my epilepsy and I said no that I needed a Neurologist. I'm glad that I did get one because I'm getting a lot done now.
